Is the tattoo establishment in a non-residence? If No: Is it separated from the living quarters by a solid permanent partition? Is there a solid door between the residence and the establishment? (must remain closed during business hours) Is there a direct outside entrance to the establishment? 2. Are tables, chairs, etc., constructed of impervious or smooth and easily cleanable material? 3. Is there a sink for handwashing with hot and cold running water available in the tattoo area? 4. Are toilet facilities available? 5. Is the establishment at least 150 square feet in area? Is it adequately lighted and ventilated? Is it clean, sanitary and vermin free? 6. Are floors in immediate tattoo area impervious, smooth and washable? 7. Are instruments, dyes, pigments, stencils, tattoo machines, and equipment stored in closed cabinets? 8. Is smoking and consumption of food or drink prohibited in the area where the actual tattoo procedure is performed? 9. Are ink cups and dyes or inks for single patron use? 10. Are tubes, needle bars, needles and instruments sterile, single patron use and disposable? If No: Are items cleaned with a detergent according to manufacturer’s recommendations? Are items steam sterilized (250 degrees F/121 degrees C for 15 minutes, at 15 psi)? Are items dry-heat sterilized (350 degrees F/170 degrees C for 1 hour)? 11.
