HOW TO ENTER: Official Entry Forms for the Teen Life Of Style Crew Casting Call (the Competition) will be available from June 29 2007 through July 22 2007 at the following locations: Guest Services at Lakewood Center 500 Lakewood Center Mall (Lakewood Blvd. and Del Amo Blvd.) Lakewood California ("Lakewood Center"); Guest Services at Los Cerritos Center at 239 Los Cerritos Center Cerritos California ("Los Cerritos Center"); and Guest Services at Stonewood Center 251 Stonewood Street (Lakewood Blvd. and Firestone Blvd.) Downey California ("Stonewood Center")(Lakewood Center Los Cerritos Center and Stonewood Center are collectively the "Centers"). You can only enter the Competition by submitting an Official Entry Form in person and having your photo taken (a) at Center Court at Lakewood Center from 12 noon to 4 p.m. (local time) on July 20 2007 (b) at Mervyn's Court at Los Cerritos Center from 12 noon to 4 p.m. (local time) on July 21 2007 or (c) at Macy's Court at Stonewood Center from 12 noon to 4 p.m. (local time) on July 22 2007. Only persons who enter in accordance with these Official Rules will be permitted to participate in the Competition and be eligible for a prize. Any other attempt at entry is void. Complete the Official Entry Form and follow the instructions providing all mandatory information. The parent or legal guardian of each entrant who is a minor must execute the Official Entry Form. Incomplete entries or failure to follow the instructions procedures or these Official Rules shall render the entry invalid. There is a limit of one (1) entry per person. If multiple entries are received from any one person only the first entry will be considered and subsequent entries will be disqualified. 2.
