Content Inside: No Purchase Necessary Void Where Prohibited By Law TEEN LIFE OF STYLE CREW CASTING CALL OFFICIAL ENTRY FORM Enter for a chance to win a $250 One Great Gift Card together with the opportunity to work part time on an "at-will" basis (up to 20 hours per month) from August 2007 through August 2008 for $8.00 per hour as a member of the Teen Life Of Style Crew and also possibly as the Southern California Teen Lifestyle Spokesperson. You must submit this Official Entry Form in person and have your photo taken (a) at Center Court at Lakewood Center from 12 noon to 4 p.m. (local time) on July 20 2007 (b) at Mervyn's Court at Los Cerritos Center from 12 noon to 4 p.m. (local time) on July 21 2007 or (c) at Macy's Court at Stonewood Center from 12 noon to 4 p.m. (local time) on July 22 2007 (Lakewood Center Los Cerritos Center and Stonewood Center are the "Centers"). Up to twenty-five (25) semi- finalists will be selected based on the following criteria: who is the most style savvy; who has significant community involvement; who has significant school involvement; who has the most fashion awareness through magazines and television; and who makes the best appearance for purposes of media coverage. Each semi-finalist will be required to sign an Affidavit/Release and if the semi-finalist is under 18 years of age the semi-finalists parent or legal guardian must sign an Affidavit acknowledging the Affidavit/Release. The semi-finalists will participate in the Teen "Life Of Style" Challenge in which each semi-finalist will be given a $150 One Great Gift Card to spend on a "style" during a two (2)-hour shopping period at Los Cerritos Center followed by a modeling and interview session in which the semi- finalists each will have approximately one (1) minute to model their styles explain where they shopped why they chose their style and how much they spent and approximately five (5) minutes to respond to additional questions.
