Content Inside: The Hip-Hop Boot Camp 2007 All information contained on these forms will be kept confidential. 1 Camp Information Sheet Dates: July 29 – August 2, 2007 – Grades 7-12 Location: The 2007 Hip-Hop Boot Camp will be held at Camp Virgil Tate in Charleston, West Virginia (map enclosed) Cost - $50.00 (includes food, lodging, insurance, all classes, and basic camp administrative costs) – applications returned by the early registration date (JUNE 8, 2007) will receive $10.00 off the cost of camp. If you qualify for free or reduced meals, there is also a $28 scholarship to camp. If there are extenuating circumstances and a youth needs a special assistance scholarship to attend camp, please contact Paul Henderson at (304) 344-2764. There will be a few scholarships available. No refunds will be given for campers who leave camp after registering, with the exceptions of serious illness or a death in the immediate family. CAMP PRE-REGISTRATION INFORMATION SUMMARY • All applications should be accurately completed and returned with health statement and any other forms. Due to the fact that camp participation is limited, any delays during the pre-registration process could keep your teen from attending camp. Please make sure that all questions are answered, all forms have the required signatures, and everything is returned in a timely manner. • There will be NO money down. Please do not send money for the Hip-Hop Boot Camp with your pre-registration form. You will receive your camp bill in the second camp mailing packet. Any individual scholarships and the early registration bonus will be deducted from the cost. • There is a $10 cancellation fee if you do not show up and do not cancel your reservation. If you register and find that you cannot attend camp, you should call Paul Henderson (344-2764) at least one week prior to camp in order to cancel the registration.
